ACCREDITATION STATEMENT:

Henry Ford Health is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

DESIGNATION STATEMENT:

Henry Ford Health designates this live course for a maximum of 20.5 AMA PRA Category 1 Credit(s) TM. Physicians should only claim the credit commensurate with the extent of their participation in the activity.

“Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn:

– up to 20.5 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program; and

–  up to 20.5 credit toward the CME requirement(s) of the American Board of Surgery’s (ABS) Continuous Certification program.

It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM or ABS MOC credit.

There is a 30-day post-event evaluation completion deadline to receive your CME/MOC credits.”

After completing the evaluation click the Claim MOC button to finalize the submission & then ensure your CloudCME profile reflects the following required details so we can correctly transmit your credits:

  • Click Yes to Claiming MOC & enter your Birth Month & Birth Day
  • Then select the MOC Board & Enter your Diplomate ID (for ABIM) or Learner ID (for ABS)
  • Enter your License & NPI # (if applicable)
  • You must have the option selected at the bottom of your profile to provide permission for us to share & transmit your CME/MOC data to ACCME
